INCLUDED DATE: Each section of the form should be completed in accordance with known information at the time the inquiry or order is originated. The description of the machine or process equipment should be equivalent to the title block on drawings and include the part or component being processed. The name of the individual listed for technical information should be that of the lubrication engineer, or other qualified person designated to review preliminary and final. drawings.
