A Failure Reporting Form, figures 1 and 2, has been developed by
the IEEE Switchgear Committee to improve the reliability of circuit
breakers through uniform reporting of field failures. The use of
this approach is the recommended practice for all users of power
circuit breakers.
Purpose The use of the Failure Reporting Form, included in this document, is recommended to standardize reporting of field failures and to use these experiences to improve the reliability of power circuit breakers
